Risk Factor Management Program

Diabetes prevalence in Australia is growing, and there is now very good evidence that certain risk factors directly impact on the likelihood of a person developing diabetes. You can take action now to manage many of these risk factors through lifestyle changes.


What is the Risk Factor Management program?

The Risk Factor Management program is for people with multiple risk factors for the development of type 2 diabetes.

It uses the Australian Type 2 Diabetes Risk Assessment Tool (AUSDRISK), which determines a risk score that predicts the likelihood of an individual developing type 2 diabetes within the next 5 years. The program aims to help members reduce their risk score and the chance of developing type 2 diabetes. The AUSDRISK tool was developed by the Baker IDI Heart and Diabetes Institute on behalf of the Australian Government.

How does it work?

Through a series of phone-based consultations, your health coach will help you become an expert regarding your own health. This will include familiarising yourself with the recommended tests, measurements and check-ups, knowing when they should be scheduled and what they mean for you, while also helping you understand and manage any medications you may be taking.

This program is coordinated by our health partner Remedy Healthcare, an experienced provider of home and community based care.

Are you at risk of developing type 2 diabetes?

Take the AUSDRISK risk test or answer the 5 simple questions below to estimate your risk of developing type 2 diabetes:

  • Are you aged 45 or over?
  • Do you have a waist measurement greater than 100cms for men and 88cms for women?
  • On average, do you do less than 2.5hrs of physical activity/week?
  • Are you taking medication for high blood pressure?
  • Have you ever been found to have high blood glucose (blood sugar)?

If you answered “yes” to more than 2 of the above questions you may be at risk of developing type 2 diabetes. These risk factors for type 2 diabetes can be reduced and our useful program can help you achieve your health goals while reducing these risks.


Are you eligible for the Risk Factor Management program?

To access the Risk Factor Management program, you need to

  • hold an appropriate level of private hospital cover with Australian Unity
  • have held that level of cover for at least 12 months
