A Home Care Package is a flexible form of government funding, that puts you in control of your home care services. It can fund extra support you might need to remain living independently at home, and feeling confident and connected to your community.
You’ll be allocated a pool of funding, based on your approved Home Care Package level.
Freedom to choose which services your funds are spent on, depending on your assessed needs and goals.
You’ll have a local service coordinator who will help set up your care plan and review it with you during the year to ensure it is still in line with your goals.

My Aged Care will contact you with a time and date for your assessment. This will determine if you're eligible for a Home Care Package and which level of funding meets your support needs.
You’ll receive a letter from My Aged Care to confirm the outcome of your assessment. If approved you’ll be placed on the national waitlist until your package becomes available.
Once your Home Care Package is assigned, you can choose a provider like Australian Unity to deliver your services. We’ll work with you to develop a plan and package of services to meet your needs and goals.
