and what you feel you need to make the most out of your life.
All of this will provide us with a good indication of how to
tailor a home care package to best suit you.
Once you have spoken with us, you will be ready to complete
your Aged Care Assessment.
Arrange a free ACAT assessment through My Aged Care
What is an ACAT assessment?
An Aged Care Assessment Team (ACAT) assessment is your way
to access the aged care system. It is used to determine your
eligibility for a home care package and what level of funding
you require based on your circumstances and support needs.
To complete your assessment, you will need to contact My Aged Care,
the Commonwealth Government body responsible for all aged care services.
How do I make Australian Unity my Home Care package provider?
During your assessment, My Aged Care
will provide you with a list of service providers in your
area that offer home care packages. Be sure to let them know
that you would like Australian Unity to be your provider so
they can register your package with us.
Once you have completed your assessment, you will be ready
to develop your customised Home Care Support Plan and
Agreement with Australian Unity.
Develop and accept your Home Care Support Plan and Agreement
Once you have received your ACAT approval, you will need to
get in touch
with us to organise your Home Care Agreement, which sets out how Australian Unity will
provide services. During this time, we will also work with you
to tailor an individualised support plan and budget just for you.

We'll then sit down together and take a close look at exactly
how your needs will be met. Here are some points to consider
as you develop your plan.

Working out a budget for your package
Once your support plan has been finalised, we will then review how your funding package will cover it.
Once you are happy with your support plan and budget, simply accept and sign your
Home Care Agreement and start making the most out of your life.

When you reach this stage of the process, the hard work is over. Now it’s time to relax and start using and benefiting from your services.
Your support plan is designed to assist you as your needs evolve and can be added to or cancelled at any point. The aim is to support you to enjoy your life and to thrive, whatever that means to you.
