Organising home health and care services is easy, with just 4 simple steps. Whether you’re applying for government assistance such as a Support at Home or are ready to set up self-funded care, we're here to help you.

Step 1

Start the Conversation

Begin with a simple, no-pressure chat with our expert Health Concierge. In this conversation, you can:

Identify where we can support you or your loved one
Get clear guidance on funding pathways available to you.

Step 2

Get assessed

Apply for an assessment for Support at Home through My Aged Care.
Connect with My Aged Care through our simple referral service - we'll handle the details for you.
My Aged Care will organise to visit your home to assess your needs.

Step 3

Assessment outcome

You'll receive an outcome letter from My Aged Care to let you know that you’re eligible.
You will be placed on the National Waitlist until funding is assigned to you.
While you are waiting, you can explore other support options

Receive your package and start services

When your funding letter arrives, give us a call—we're ready to begin.
Together we will continue to create your personalised support plan that meets your needs and goals.
