Restorative care provides support to help you regain your strength and abilities after an accident or illness, or adapt to new or changing age-related health conditions.
What does it offer?
Up to 16 weeks of intensive allied health and/or nursing services aligned to your needs.Each episode of restorative care provides funding of up to $6,000.How to apply
You'll have an assessment by My Aged Care to see what you need.You can apply for the Restorative Care pathway even if you're not receiving care under the Support at Home program.If you’re already receiving care services, you can apply for restorative care alongside your ongoing services.
The Assistive Technology and Home Modifications (AT-HM) Scheme supports you to live safely and independently at home. It provides equipment such as walking frames, and home modifications such as ramps and grab handles.
What does it offer?
Funds of up to $15,000 to cover the cost of assistive technology or home modifications.If eligible, you'll be approved at one of three funding tiers depending on your needs.Funds are valid for 12 months.How to apply
You'll have an assessment by My Aged Care to see what you need, and may be approved for funding for assistive technology or home modifications, or both.We can help you with organising your assessment and answer any questions you have.
Our end-of-life pathway supports you or a loved one to stay at home for as long as possible, with dignity and comfort. We provide compassionate care for the final stages of life such as personal and domestic care, and in-home nursing.
What does it offer?
Up to $25,000 over 12 weeks.Available for people who have been diagnosed with three months or less to live.How to apply
If you’re already receiving care services, your provider can help you to transition to this pathway.If you’re not currently receiving home care services, your GP or My Aged Care can help you apply.We can answer questions you may have, and can connect you to My Aged Care to organise services.
