You can apply for care through the Restorative Care pathway even if you're not receiving care through other programs like Support at Home.
Step 1
You'll need an Aged Care Assessment, which can be arranged either online or over the phone.
We can help guide you through this process if you're unsure where to start.
Step 2
When your funding is assigned, you're ready to choose a restorative care pathway provider.

Your Restorative Care Partner will create a personalised plan to make the most of your funding.
Together, we'll start your services at home - supporting you to rebuild strength, confidence and independence at a pace that feels comfortable.
