Now that you have been assigned a Home Care Package, you’re probably eager to get started with your new home health and care services. Once your package has been assigned you have 56 days to choose your provider and set up your services, so there’s no time to waste.
Getting started with services is easy, just a few simple steps:
Step 1
Once you have been allocated a Home Care Package, you will receive a package assignment letter. Call us on 1300 160 170 when you get this letter and we will organise your services.
You’ll have 56 days from the date you receive your package assignment letter to commence services. However, an extension of 28 days is available, give us a call and we will be happy to help.
Step 2
We will start planning your support services with you, so that these services are ready to go when your package becomes available. You can have a look at our services ahead of time to get an idea of what you might want to ask for.
Step 3
You will meet your Care Planner, who will get to know you and work with you to design a tailored care plan that meets your needs and goals, and makes the most of the funding you have.
Step 4
Once your care plan is set up, you can sit back and relax. Your services will begin soon and you can get on with doing the things you love.
