About Home Care Packages

Home Care Packages offer comprehensive and flexible health and care services to support you or a loved one to stay confidently living at home.

So if you or a loved one needs a little help around the home, support getting out and about, or allied health services, a Home Care Package could be a good option.

How to apply for a Home Care Package

It’s easy to get started, and we’re here to help every step of the way.

Step 1

Start the conversation

Begin with a simple chat with our care team. In this conversation, you can get guidance on services, funding options, and the application process.

Step 2

Get Assessed

You can apply for an assessment through My Aged Care. We can help you with your application, just give us a call.

Step 3

Assessment outcome

You'll receive an outcome letter from My Aged Care to let you know if you’re eligible. You will be placed on the National Waitlist until funding is assigned to you.

Step 4

Receive package and start services

When your funding letter arrives you can choose your provider. Just give us a call and we can get you started with your tailored care plan.

Important information

From November 2025 Home Care Packages are being replaced by the new Support at Home Program

If you are applying for government assistance through My Aged Care don’t worry, you can still apply by following the steps above.

What is changing with Support at Home?

From 1 November 2025, the Australian Government is introducing a new aged care program called Support at Home. It will replace the Home Care Package and Short-Term Restorative Care programs.

Support at Home is designed to offer:

More consistent and flexible care
Clearer funding
Simpler access to services

What will happen to my existing Home Care Package?

If you are already on a Home Care Package or have been assessed and are waiting for your package to be assigned, you will transition to the Support at Home program from November 2025.

How this transition will work for you depends on when you were assessed and started to receive your Home Care Package. The good news is that you don’t need to do anything, and we are here to answer your questions and guide you through this change.
