People receiving a Support at Home quarterly budget will be able to apply for Restorative Care Pathway funding if it is clinically recommended. However, participants who have previous HCP unspent funds carried forward may need to exhaust their funds before applying for Restorative Care Pathway funding.
These are some of the features of the Restorative Care Pathway: An episode of restorative care provides up to 16 weeks of tailored allied health and nursing services, with funding of up to $6,000. It includes restorative care management activities funded by the Restorative Care budget.

A restorative care goal plan complements the ongoing Support at Home care plan, and an exit plan is required to outline next steps after the Restorative Care episode concludes. If necessary, a Support Plan Review can request additional funding or reassessment for other aged care services.
We will work with you to identify opportunities where Restorative Care activities could assist you to regain or maintain your functional abilities and skills. Investing in Restorative Care programs can have a significant positive impact, so please reach out to us to discuss your options.
