This is a complex topic, and often a difficult time for people caring for someone who is nearing the end of their life, so careful and timely supports and referrals are essential to support this process in a streamlined and dignified way. Eligible participants can access the End-of-Life Pathway, which provides funding of up to $25,000 and must be used within 12 weeks, with a possible 4-week extension if required.

It is intended to provide additional in-home aged care services (such as personal care, domestic assistance and general nursing care) to complement services available under State and Territory-based palliative care schemes.
There is no option for further funding beyond the maximum of 16 weeks. Participants will have their ongoing Support at Home quarterly budget suspended whilst receiving the End-of-Life pathway funding. If required, they can resume their previous care and services using their quarterly budget, or be referred for a Support Plan Review if their care needs exceed their available budget.
