Medicare and Reciprocal Health Care Agreements
Medicare is the publicly funded universal health care insurance scheme in Australia. The scheme either partially or fully covers the cost of most health care, with services being delivered by state and territory governments or private enterprises.
Enrolment in Medicare is not just for Australian citizens and permanent residents. International visitors from 11 countries that have Reciprocal Health Care Agreements are partially or fully covered under Medicare for medically necessary treatment.
The Reciprocal Health Care Agreements allow eligible visitors to access essential medical care while in Australia—and at the same time enable Australians to receive similar care when visiting these countries.

Do you earn income in Australia?
If you're an overseas visitor covered under a Reciprocal Health Care Agreement and earning income in Australia, you may be subject to the Medicare Levy Surcharge (MLS)—just like Australian residents. The MLS is an additional tax applied to higher-income earners who do not hold eligible private hospital cover.
Important
Having an Overseas Visitors Health Cover (OVHC) does not exempt you from the MLS
If your tax advisor or another professional has informed you that you're liable for the surcharge, you may be able to avoid it by purchasing a Complying Health Insurance Policy (CHIP) with Australian Unity.
Do I need both an OVHC and CHIP product?
What’s best for you really depends on your personal circumstances and what level of cover you’re looking for:
- CHIP: This type of cover is designed to meet the tax requirements for MLS exemption. It does not provide access to medical or hospital services to overseas visitors, as it’s intended for Australian residents only. Looking to avoid MLS? Get a quote
- OVHC: If you need actual medical or hospital cover beyond what a Reciprocal Health Care Agreement provides, you’ll need to purchase OVHC separately.
The good news?
You can choose the lowest-cost CHIP hospital cover from Australian Unity to meet the MLS exemption requirement. Plus, depending on your income, you may be eligible for the Australian Government Rebate on private health insurance, which can help reduce your premiums. Be sure to have your Medicare card handy when applying for the rebate - it's required as part of the process.
