As you were assessed between 12 September 2024 and 31 October 2025, you're a Transitioned participant.
As you've already been approved for a Home Care Package, you'll transition to the new Support at Home program from 1 November 2025.
How this will affect you will depend on the date your Home Care Package was approved. You can find this date on your approval letter from My Aged Care, or by calling them on 1800 200 422.
As you were assessed on or before 12 September 2024, you're a Grandfathered participant, even though you're not currently receiving a Home Care Package.
You'll transition to the new Support at Home program starting 1 November 2025.
As you were approved for a Home Care Package on or before 12 September 2024, you're a Grandfathered participant.
You'll continue to receive your Commonwealth Home Support Program (CHSP) services until your new Support at Home package is assigned.
As you were approved for a Home Care Package between 12 September 2024 and 31 October 2025, you're a Transitioned participant.
Your CHSP services will continue for now.
