How do I apply for the Commonwealth Home Support Programme (CHSP)?
1Contact Australian Unity
The first step is to contact us and tell us a bit about yourself. We will ask you about your health, what you are looking for and what you feel you need to live your best life. All of this will give us a good idea of how we can support you and which of our services will best suit you.
We understand that this can be a difficult time and our friendly and experienced team will support you and guide you through each step of the process and we will be available to answer any questions you have.
Once you have spoken with us, you will be ready to complete your assessment with My Aged Care.
2Receive a free assessment through My Aged Care
This is conducted through My Aged Care and used to establish what support you might need to remain living independently in your own home. Together, you and your assessor will evaluate your eligibility, look at the range of services which could assist you and how much they will cost.
My Aged Care will contact you with the outcome of your assessment. If approved for the CHSP, your next step is to contact us so we can arrange a quote for your services.
3Accept your quote from Australian Unity
Once you have selected us as your provider, we will work with you to establish how often you require the services and how much you will be required to pay.
The cost of your services will be split into two parts
The CHSP subsidy
This is the amount the government pays to subsidise your services. These funds are sent directly to us.
Potential additional fee
You may be required to pay an additional fee and this will be determined by your income test as part of your assessment.
Once you are happy with your quote, it is time to accept the agreement.

4Create a personalised care plan
We will work with you to create your personalised care plan that’s tailored to your needs and goals, to ensure you receive the services that are important to you. Your personalised care plan is designed to assist you as your needs evolve and can be added to or cancelled at any point.
Once your care plan has been set, you can start using and benefiting from your CHSP services.
