Doctor health checks, for regular good health
Having health measures such as your blood pressure checked or your waist size properly monitored during a check up with your GP is all part of a good health regime, especially if it’s done on a regular basis.
Let us help you keep those visits regular. We’ll give you up to $150 per person per calendar year on recognised health checks performed by a doctor in private practice where you are unable to claim through Medicare.
Be sure to download a doctor health check form for your GP to complete during your visit. Then, when you’re ready to claim, simply return the tear-off section along with your medical bill and a completed claim form to Australian Unity. Keep the other part of the form with your personal health information for your own reference.

Available with all Hospital, Combination and Extras covers except Care 'n Repair, Budget Hospital and Overseas Visitors Cover. Where a member has both Hospital and Extras cover benefits may only be claimed under the Hospital cover
